Residents with Health Insurance (including Medicare / Medicaid plans)

Choices accepts most insurances and New York State Medicaid Plans upon verification. Many insurances cover surgical abortion and medical abortion (the abortion pill). Our board-certified OB/GYN doctors are in-network with most insurance plans. If you have a commercial insurance (Non-Medicaid) you may apply for confidentiality reasons if the insurance is under a Parent or Spouse or if it does not cover Elective Abortion.

Residents without Health Insurance

Choices Women’s Medical Center provides New York State Emergency Medicaid to cover pregnancy-related services including abortion. You can apply on-site and be approved the same day as your appointment.

Schedule an abortion appointment online, or by calling 718-786-5000. On the day of your appointment, please bring a valid government-issued, non-expired ID and a dated proof of residence in the last 30 days. Examples of proof of residence are a utility bill or letter addressed to your home address (no PO Box accepted).

Choices Women’s Medical Center provides New York State Emergency Medicaid to cover pregnancy-related services including abortion. You can apply on-site and be approved the same day as your appointment.

Please bring the following original documents with you:

  • Valid photo ID (must be government issue and cannot be expired)
  • Birth Certificate or Passport (if your Birth Certificate is in a foreign language, you must have a notarized translation to English)
  • Social Security Card (if applicable)
  • Proof of Address (must be under the patient’s name and dated within 90 days. PO Box addresses are not accepted)
    • Postmarked, non-window envelope
    • Lease with address
    • Address and date printed on magazine ( cannot be a stick on label)
    • Utility Bill
  • Proof of Income OR, if you are a full-time student under the age of 21, Proof of Enrollment
    • If a patient receiving income weekly, their 4 most recent & consecutive pay stubs are required. If receiving income bi-weekly , their 2 most recent & consecutive.
    • If receiving unemployment, the most recent print-out of their benefit is needed.

Unfortunately, you do not qualify to apply for Emergency Medicaid based on your income. However, Choices partners with funding agencies that can help cover the cost of abortion services or we can help you work out a self-pay plan. The self-pay cost of abortion varies from $500 to $3,000, depending how far along you are in your pregnancy. Out of State Patients

Choices accepts Medicaid from New Jersey, Maryland, Pennsylvania, Virginia and Delaware. Choices will help obtain prior authorization, if required. If you are planning to use out of state Medicaid, please contact 718-349-9100 EXT 520 a week before your scheduled appointment.

GYN Care

Residents without Health Insurance

Choices Women’s Medical Center provides enrollment onsite Family Planning Benefit Program to cover family planning services. You can apply on-site and be approved the same day as your appointment.

Family Planning Benefit Program (FPBP) Requirements:

**PATIENT MUST BE U.S. CITIZEN, PERMANENT RESIDENT, OR TEMPORARY NON-IMMIGRANT (SHORT-TERM VISA HOLDER)**

PATIENT CANNOT HAVE ANY HEALTH INSURANCE WHETHER THEY ARE THE PRIMARY HOLDER ON THE POLICY OR A DEPENDENT

PATIENT MUST RESIDE WITHIN THE 5 BOROUGHS

Documents needed:

  • Valid photo ID: Must be government issued, cannot be expired.
  • Birth Certificate or Passport: Birth Certificates in a foreign language must have a notarized translation in English.
  • Social Security card
  • Proof of Address: Must be under patient's name & dated within 3 months/90 days, NO P.O. Box.
  • Proof of Income:
    • If patient is receiving income weekly, they are required to provide four (4) most recent & consecutive pay stubs. If patient is receiving income bi-weekly, they are required to provide two (2) most recent & consecutive pay stubs. Pay stubs must be up-to-date.
    • If patient is unemployed and does not receive any income, a letter of support written by their supporter is needed along with supporter's proof of income. (Same proof of income guidelines as patient}.
    • If patient is receiving unemployment, the most recent print-out of their benefits is needed.
    • If patient is married, a marriage certificate or license is required as well as spouse's proof of income. (Same proof of income guidelines as patient).
    • Birth Certificates of the patient's children are needed if patient is receiving any type of income.
