Choices provides FREE GYN services through our Family Planning Benefit Program to residents of New York State who are US Citizens or permanent residents, can have children, meet income guidelines and do not qualify for Medicaid or Family Health Plus.

Through the program, patients have FREE access to all forms of birth control (including pills, condoms, diaphragms, and the patch), emergency contraception, as well as information about their choice of birth control and family planning. During a family planning visit, patients will have check-ups and pap smears, and can have laboratory tests related to family planning services (pregnancy testing), HIV testing, and sterilization (ESSURE [link]).

To see if you qualify, and to receive same-day services, please bring the following documents to your appointment:

• Valid photo ID (must be government issued and cannot be expired)
• Birth Certificate or Passport (if your Birth Certificate is in a foreign language, please have a notarized translation to English)
• Social Security Card
• Proof of Address (in your name and dated within three months)
• Proof of Income
